Care That Wraps Around Your Whole Family
Six disciplines of expert care working in harmony — so your loved one finds comfort, and you find peace of mind.
Six disciplines. One team.
All covered by Medicare at no cost to your family.
Skilled Nursing Care
Experienced registered nurses provide comprehensive medical care in the comfort of your home — pain management, symptom control, wound care, and medication management. Our nurses are specially trained in hospice and palliative care, ensuring your loved one receives expert attention with warmth and compassion.
Team: Registered Nurses (RNs) and Licensed Vocational Nurses (LVNs)
Pain & Symptom Management
Our medical team specializes in controlling pain and managing symptoms like nausea, shortness of breath, anxiety, and fatigue. Using a combination of medications, positioning techniques, and complementary therapies, we ensure your loved one is as comfortable as possible — so they can focus on living, not suffering.
Team: Physicians, Registered Nurses, and Pharmacist Consultants
Emotional & Spiritual Support
Our chaplains and counselors provide a compassionate presence for patients and families navigating the emotional and spiritual dimensions of end-of-life. Whether your loved one draws comfort from faith, nature, music, or quiet reflection, we honor their beliefs and respect their unique journey.
Team: Chaplains, Counselors, and Trained Volunteers
Personal Care Assistance
Our trained home health aides help with the activities of daily living — bathing, dressing, grooming, and light housekeeping — always with dignity and respect. They provide not just physical assistance but genuine companionship, building warm relationships with patients and families.
Team: Certified Home Health Aides (HHAs)
Social Work Services
Licensed social workers connect families with community resources, assist with advance directives and care planning, provide emotional counseling, and help navigate insurance and financial questions. They serve as advocates for the patient and family throughout the hospice journey.
Team: Licensed Clinical Social Workers (LCSWs) and MSWs
Bereavement Support
Grief doesn't end with loss. We provide bereavement support for families — one-on-one counseling, support groups, memorial services, and community grief resources. Our bereavement services are also available to anyone in the community who has experienced a loss.
Team: Bereavement Counselors, Social Workers, and Chaplains
"No one should have to face the end of life without a team of people who genuinely care."
Four Levels of Hospice Care
Medicare defines four levels of hospice care to ensure patients receive the right intensity of support at every stage. We provide all four.
Routine Home Care
The most common level. Your care team visits regularly in the comfort of home — nursing, aides, social work, chaplaincy, and volunteer support.
Continuous Home Care
During a medical crisis, we provide continuous bedside nursing — up to 24 hours a day — until symptoms are stabilized and comfort is restored.
Inpatient Respite Care
Short-term care (up to 5 days) in a facility to give family caregivers a needed break. Your loved one receives the same attentive care.
General Inpatient Care
For symptoms that can't be managed at home, patients receive hospital-level care. Once stabilized, they return to their preferred setting.
What's Included at No Cost
Under the Medicare Hospice Benefit, all of the following are provided at no cost for care related to the hospice diagnosis.
